Signing up for HealthShare Referral Manager (HSRM) as a
community provider

Are you interested in using HSRM?
Keep reading to find out how you can sign up!

First, make sure to confirm you / your facility meets the following criteria:

Your facility has an active network agreement

(i.e. TriWest or Community Care Network (CCN) or Veterans Care Agreement (VCA)

A point of contact from your facility...

...has reached out to the VA Medical Center Community Care Point of Contact(s) to express interest in HSRM and obtained concurrence with the VA Medical Center to move forward

Next, you'll do the following 5 steps:

Step 1

Training

Each staff member attends two-hour training on VHA Train or reviews Community Provider User Guide

This is for All Staff

Step 2

ID.ME

Each staff member creates an ID.me account at https://www.id.me/

This is for All Staff

Step 3

Submit Team Information

One staff point of contact fills out the HSRM End User Tracker (EUT), then sends it to the HSRM Help Desk at [email protected]

This is for the facility point of contact

Step 4

Receive Accounts

HSRM Help Desk will create accounts in HSRM, then provide confirmation of account creation to the facility point of contact

Step 5

Log Into HSRM

Go to https://ccracommunity.va.gov to log into HSRM and begin processing

This is for All Staff

Note: Only staff members who process VA referrals and authorizations or communicate directly with the VA Medical Center should create HSRM accounts.

Important: The staff email addresses provided in the End User
Tracker must match the email addresses your staff used to register for
ID.me

Note: If your organization has already been provisioned and a new staff member needs an HSRM account, the community provider facility POC must open a ticket with the HSRM Help Desk.
